**Join Our Dynamic Team as an ED Mental Health/RN Case Manager!****Job Description:**- Are you ready to make a significant impact in the field of emergency nursing and mental health?- We are seeking an enthusiastic Nurse Case Manager for the Emergency Department to join a vibrant and diverse healthcare team!**Responsibilities:**- Collaborate with patients, families, and a multidisciplinary team to ensure seamless care across the healthcare continuum.- Assess patient needs and determine the most appropriate level of care, from outpatient to inpatient settings.- Engage in effective discharge planning and facilitate referrals to various levels of care.- Utilize your knowledge of InterQual Criteria to evaluate patient status and medical necessity for admissions.- Work closely with advanced practice clinicians and physicians to propose and implement safe discharge plans.- Coordinate care for various patient populations, including those facing mental health challenges and chronic pain.- Maintain professional relationships with community resources and payers to enhance patient outcomes.**Qualifications:**- Education: Associate's degree in Nursing.- Licensure: Valid, unrestricted Professional Nursing License in any U.S. state, Puerto Rico, or a territory.- Certification: Current Basic Life Support (BLS) certification by the American Heart Association; Case Manager Certification (CCM or ACM) within 2 years of hire.- Experience:- Minimum of 2 years of care/case management experience in an Emergency Department setting.- 2 years of patient care experience (Hospital, Home health, Skilled Nursing).- Proficiency in Electronic Medical Records, Outlook, Microsoft Office Word, and Excel.- 1 year of travel experience is a must.**Shift Information:**- Work schedule: Sunday - Saturday, 10 shifts, variable 7 am - 5:30 pm with weekend rotation and call responsibilities.**Why Catalytic Solutions (CatSol):**- At CatSol, we connect dedicated therapists with rewarding assignments across the country.- Enjoy competitive pay, seamless onboarding, and a supportive recruiter who's with you every step of the way.- Join us — where your expertise makes a difference every day!
